Предполагается, что Zabbix уже установлен и работает, пользователь MySQL – zabbix есть.

Даём ему права на просмотр MySQL:
MariaDB [(none)]> grant usage on *.* to 'zabbix'@'localhost'; Query OK, 0 rows affected (0.00 sec)
В файле /etc/zabbix/zabbix_agentd.conf проверяем наличие строки:
Include=/etc/zabbix/zabbix_agentd.d/
Проверяем наличие или создаём файл:
# ls -l /etc/zabbix/zabbix_agentd.d/ total 4 -rw-r--r-- 1 zabbix zabbix 1517 Aug 28 10:12 userparameter_mysql.conf
Содержимое файла по-умолчанию:
# cat /etc/zabbix/zabbix_agentd.d/userparameter_mysql.conf
# For all the following commands HOME should be set to the directory that has .my.cnf file with password information.
# Flexible parameter to grab global variables. On the frontend side, use keys like mysql.status[Com_insert].
# Key syntax is mysql.status[variable].

Создаём каталог, указанный в переменной HOME:
# mkdir /var/lib/zabbix
И в нём создаём файл .my.cnf (точка нужна) с таким содержимым:
# cat /var/lib/zabbix/.my.cnf [client] user = zabbix password = p@ssw0rd
Перезапускаем zabbix-agent:
# service zabbix-agent restart Shutting down Zabbix agent: [ OK ] Starting Zabbix agent: [ OK ]
И проверяем данные:
